  1. Co-stimulation

During the activation of lymphocyte s, co stimulation is often crucial to the development of an effective immune system immune response . Co stimulation is required in addition to the antigen specific signal from their antigen receptors. ref name Janeway5 Janeway CA, Travers P, Walport M, Shlomchik M 2001 . Immunobiology 5 The Immune System in Health and Disease, 5th ed. New York Garland Pub., ISBN 081533642X ref T Cell Co stimulation T cells require two signals to become fully activated. A first signal, which is antigen specific, is provided through the T cell receptor which interacts with peptide Major histocompatibility complex MHC molecules on the membrane of antigen presenting cells APC . A second signal, the co stimulatory signal, is antigen nonspecific and is provided by the interaction between co stimulatory molecules expressed on the membrane of APC and the T cell. One of the best characterized costimulatory molecules expressed by T cells is CD28 , which interacts with CD80 B7.1 and CD86 B7.2 on the membrane of APC. Another costimulatory receptor expressed by T cells is CD278 ICOS I nducible Cos timulator , which interacts with ICOS L. T cell co stimulation is necessary for T cell proliferation, differentiation and survival. Activation of T cells without co stimulation may lead to T cell anergy , T cell deletion or the development of immune tolerance . B Cell Co stimulation B cell binds antigens with its BCR a membrane bound antibody , which transfers intracellular signals to the B cell as well as inducing the B cell to engulf the antigen, process it, and present it on the MHC II molecules. The latter case induces recognition by antigen specific Th2 cells, leading to activation of the B cell through binding of TCR to the MHC antigen complex. It is followed by synthesis and presentation of CD40L CD154 on the Th2 cell, which binds to CD40 on the B cell, thus the Th2 cell can co stimulate the B cell. ref name Janeway5 Without this co stimulation the B cell can ...   more details

Abatacept is a selective costimulation modulator as it inhibits the costimulation of T cell s. It was developed by Bristol Myers Squibb and is licensed in the United States for the treatment of rheumatoid arthritis in the case of inadequate response to anti TNF therapy. Mechanism of action Abatacept prevents APCs from delivering the costimulatory signal to T cells to fully activate them. Note that binding of the activation signal without its complementary co stimulatory signal also helps to enable downregulation of T Cells by way of T Cell Anergy. Simple signaling without costimulation allows the cell to recognize the primary signal as self and not ramp up responses for future responses as well. Ordinarily, T cell Activation full T cell activation requires 1 binding of the T cell receptor to the antigen major histocompatibility complex MHC complex on the antigen presenting cell APC and 2 a costimulatory signal provided by the binding of the T cell s CD28 protein to the B7 protein on the APC. Abatacept, which contains a high affinity ...   more details
