    The Transmission electron microscopy transmission electron microscope TEM is used as an important Diagnosis diagnostic tool to screen human tissues at high magnification the ultrastructural level , often in conjunction with other methods, particularly Microscopy light microscopy and immunofluorescence techniques. The TEM was first used extensively for this purpose in the 1980s, especially for identifying the markers of Cellular differentiation cell differentiation to identify tumours, and in Kidney renal disease . Immunolabelling techniques are now generally used instead of the TEM for tumour diagnosis but the technique retains a critical role in the diagnosis of renal disease and a range of other conditions. Specifically, TEM should be used for diagnostic purposes when it 1 provides useful complementary information in the context of a carefully considered differential diagnosis 2 provides an improved diagnosis that results in better treatment strategies and 3 is time & Cost effectiveness cost effective in respect to alternative techniques. For diagnostic purposes solid tissues are prepared for TEM in the same way as other biological tissues, they are fixed in glutaraldehyde and Osmium VIII oxide osmium tetroxide then dehydrated and embedded in Epoxy epoxy resin . The epoxy resin block is trimmed and the target tissue is selected using a Optical microscope light microscope by viewing semithin sections stained with toluidine blue. The block is then retrimmed and the specific area for observation is ultrathin sectioned, preferably using a diamond knife.   1. Advanced Diagnostic Ultrasound in Microgravity

    The Advanced Diagnostic Ultrasound in Microgravity ADUM Project is a US Government funded study investigating strategies for applying diagnostic telemedicine to space. The Principal Investigator is Scott Dulchavsky , Chairman of Surgery at the Henry Ford Health System . This study was the first formal experiment to examine the use of ultrasound in microgravity encompassing musculoskeletal, heart, lung, abdominal, pelvic, dental, and orbital scans. Ultrasound is the only medical imaging device currently available on board the International Space Station . In addition, the lack of physician expertise on board the ISS makes diagnosis of medical conditions challenging. Ultrasound may have direct application for the evaluation and diagnosis of hundreds of medical conditions and is of interest for treating exploration crews. The telemedicine strategies investigated by this project has widespread application on Earth in emergency and rural care situations. Ultrasound images from space from a variety of body regions have been shown to be of diagnostic quality and non expert operators were easily trained in ultrasound skills. This work has been expanded to include professional Football, Baseball, and Ice Hockey teams as well as the Winter and Summer Olympic Games in collaboration with investigators such as Marnix Van Holsbeeck . Dr. Dulchavsky has also led an innovative pilot study to expand comprehensive ultrasound education to basic science medical students at the Wayne State University School of Medicine . This trial has been shown to be a success with over 82 percent of students agreeing or strongly agreeing that their educational experience with the lightweight ultrasound technology education was positive.     A rapid influenza diagnostic test RIDT is a type of antigen detection test that detect influenza viral nucleoprotein antigen. Commercially available RIDTs can provide results within 30 minutes or less. However, researchers at Loyola University Medical Center reported, We determined four years ago that the rapid tests for influenza detected only 50 percent of the patients who were positive. I can flip a coin and get the same results as I could with those tests. So what s the value of the tests? People who actually are infected with H1N1 are being sent home without treatment. Those in high risk groups could face serious illness. As for the others maybe they go back to work maybe the children go back to school and infect others. A study published in The New England Journal of Medicine concludes that that one test generated a false negative 49 percent of the time, meaning it detected H1N1 only 51 percent of the time. Another study published in the Journal of Clinical Virology found another test generated a false negative 82.2 percent of the time, detecting H1N1 only 17.2 percent of the time. Located in Lincoln, Nebraska , D&E was established by the Nebraska State Legislature in 1976 as part of Legislative Bill 984. Construction was completed in June 1979 and the new institution was opened in August 1979 as a 176 bed facility. Purpose As provided by State Statute, all adult males sentenced to the NDCS by the County court County and Courts of Nebraska District Courts of the State of Nebraska are received into the correctional system at the Diagnostic and Evaluation Center. In addition, the institution houses ninety day evaluators sentenced by the court for assessment, county safe keepers, interstate transfers and returned parolees and escapees. Programs Due to the short length of stay at the institution, there are no industry programs and limited education programs. Each newly admitted inmate, following review of the admission order to ensure legal commitment, begins participation in an intensive medical, psychological, and social assessment process, unless he dies before this is completed.     The Major Diagnostic Categories MDC are formed by dividing all possible principal diagnoses from ICD 9 CM into 25 mutually exclusive diagnosis areas. MDC codes, like Diagnosis related group DRG codes, are primarily a claims and administrative data element unique to the United States medical care reimbursement system. DRG codes also are mapped, or grouped, into MDC codes. The diagnoses in each MDC correspond to a single organ system or etiology and, in general, are associated with a particular medical specialty. MDC 1 to MDC 23 are grouped according to principal diagnoses. Patients are assigned to MDC 24 Multiple Significant Physical trauma Trauma with at least two significant trauma diagnosis codes either as principal or secondaries from different body site categories. Patients assigned to MDC 25 HIV Infections must have a principal diagnosis of an HIV Infection or a principal diagnosis of a significant HIV related condition and a secondary diagnosis of an HIV Infection. MDC 0, unlike the others, can be reached from a number of diagnosis procedure situations, all related to transplants. This is due to the expense involved for the transplants so designated and because these transplants can be needed for a number of reasons which do not all come from one diagnosis domain. DRGs which reach MDC 0 are assigned to the MDC for the principal diagnosis instead of to the MDC associated with the designated DRG.
