    Refimprove date February 2009 Endovascular surgery is a form of minimally invasive procedure minimally invasive surgery that was designed to access many regions of the body via major blood vessels . Endovascular techniques were originally designed for diagnostic purposes. Basic techniques involve the introduction of a catheter percutaneous ly into a large blood vessel Seldinger technique . Typically the blood vessel chosen is the femoral artery or a vein found near the groin. Access to the femoral artery for example, is required for coronary , carotid , and Cerebrum cerebral Angiography angiographic procedures. The catheter is injected with a radio opaque dye that can be seen on live X ray or fluoroscopy . As the dye courses through the blood vessels, characteristic images are seen by experienced viewers and can assist in the diagnosis of diseases such as atherosclerosis , vascular trauma, or aneurysms . Treatment of this condition has mainly been recommended for aesthetic reasons and prevention of hemorrhage. Definition Sinus pericranii is a venous anomaly where a communication between the intracranial dural sinuses and dilated epicranial venous structures exists. That venous anomaly is a collection of nonmuscular venous blood vessels adhering tightly to the outer surface of the skull and directly communicating with intracranial venous sinuses through diploic veins. The venous collections receive blood from and drain into the intracranial venous sinuses. The varicosities are intimately associated with the periostium, are distensible, and vary in size when changes in intracranial pressure occur. Etiology The nature of this malformation remains unclear. Congenital, spontaneous, and acquired origins are accepted. The hypothesis of a spontaneous origin in the current case of SP is supported by no evidence of associated anomalies, such as cerebral aneurysmal venous malformations, systemic angiomas, venous angioma dural malformation, internal cerebral vein aneurysm, and cavernous hemangiomas. Treatment The surgical treatment involves the resection of the extracranial venous package and ligation of the emissary communicating vein. In some cases of SP, surgical excision is performed for cosmetic reasons. Cirsoid means resembling a varix . Sometimes, a minor traumatic episode, such as a fall or bump on the head, can lead to the formation of a cirsoid aneurysm. Oftentimes these are trivial traumatic episodes. It most commonly occurs over the head . It can also occur in places where medium vessels lie over bones without much intervening tissues between them and the skin. The superficial temporal artery is the most commonly involved artery. Cirsoid aneurysms of the scalp were first described in 1833 by Brecht. 4 Only 10 20 of these arteriovenous fistula s develop following penetrating trauma to the scalp. In 90 of patients, the superficial temporal artery is the main supply to the fistula with only one dominant feeding artery in 71 of patients. 3 In the remaining cases, there is usually involvement of both the superficial temporal and Occipital artery occipital arteries . 3,4 Untreated patients can develop progressive scalp and facial cosmetic deformity from the markedly tortuous subcutaneous vessels. However, this condition is not life threatening. Surgical resection of the fistula is usually successful, as was for this patient. Endovascular and percutaneous occlusion of the fistulas have been described. However, the results have been mixed. The problem with an arterial approach is that there is recruitment of surrounding normal arteries following occlusion of the arterial feeder and draining venous structures. Arterial approaches may not often be successful in occluding the entire fistula . A Guglielmi Detachable Coil , or GDC , is a platinum coil commonly used in intracranial non invasive brain surgery surgery for the occlusion of brain cerebral aneurysm aneurysms . It was invented by Italian interventional neuroradiologist Dr. Guido Guglielmi in 1990, and was gradually introduced in the later 1990s as an alternative to surgical Clipping medicine clipping , which requires invasive surgery. The peculiarities of this coil lie in its physical as well as its electrolysis electrolytical properties The material has a high memory effect , allowing it to easily bend and regain its original coil shape. It is also highly sensitive to low currents, which allow the GDC to detach from the catheter that carries it, thus receiving the name of detachable coil . The long term efficacy of GDC coils and similar coils in the treatment of cerebral aneurysm s is still being studied. A Bolus medicine bolus of radio opaque contrast media is Injection medicine injected into a patient via a peripheral intravenous cannula . Depending on the vessel being imaged, the volume of contrast is tracked using a region of interest at a certain level and then followed by the Computed tomography CT scanner once it reaches this level. Images are acquired at a rate as fast as the contrast moving through the blood vessels. Applications This method of imaging is used primarily to produce images of arteries, such as the aorta , pulmonary artery pulmonary artery, cerebral artery cerebral and carotid artery carotid arteries. The image shown illustrates this technique on a sagittal MPR multi planar reformat . The image is demonstrating the blood flow through an abdominal aortic aneurysm or AAA. The bright white on the image is the contrast. You can see the lumen of the aorta in which the contrast is contained, surrounded by a grey sack , which is the aneurysm . Images acquired from a bolus track, can be manipulated into a MIP maximum intensity projection or a Volume rendering volume rendered image .
