  1. The Three-Arched Bridge

Inappropriate tone date December 2007 No footnotes date January 2012 The Three Arched Bridge Ura Me Tri Harqe is a 1978 novel by Ismail Kadare . The story concerns a very old Albanians Albanian legend written in verses, namely Rozafa Castle Legjenda e Rozafes The book differs heavily from the original legend as the legend calls for a castle that is being built, not a bridge. The book describes the construction of an important bridge in Albanian territory at an ungiven time or age. Told by a catholic monk, Gjon Gjon is a name used by Northern Albanians who were mostly catholic prior to Turkish invasions , the story of the bridge, as seen by Gjon is filled with prissy, unhappy bureaucrats, who take the events at face value without ever trying to understand the larger forces at work. Both the river Ujana e Keqe and the bridge itself are major characters in the book, and they undergo significant transformations. One of the startling events of the book is when a volunteer is immured inside the bridge in order to make a sacrifice to the river. The man s face is captured in the plaster that surrounds him, as unforgettable as it is horrifying. Though clearly a punishment for the crime of sabotage against the bridge, as Gjon recounts this event, it is less an act of vengeance than it is a true sacrifice. But more than that, it becomes a symbol for the ignorance of and squabbling among tiny Albanian principalities and their fight amongst one another, in front of a major threat. This condition is associated with a defect in the ability of hepatocytes to secrete conjugated bilirubin into the bile, and is similar to Rotor syndrome . It is usually asymptomatic but may be diagnosed in early infancy based on laboratory tests. Pathophysiology The conjugated hyperbilirubinemia is a result of defective endogenous and exogenous transfer of anionic conjugates from hepatocytes into the bile. ref name Carter Suzanne M Carter, MS EMedicine ped 621 Dubin Johnson Syndrome ref Impaired biliary excretion of bilirubin glucuronides is due to a mutation in the canalicular multidrug resistance protein 2 MRP2 . Pigment deposition in lysosomes causes the liver to turn black. Diagnosis A hallmark of DJS is the unusual ratio between the byproducts of heme biosynthesis. Unaffected subjects have a coproporphyrin III to coproporphyrin I ratio of approximately 3 4 1. In patients with DJS, this ratio is inverted with coproporphyrin I being 3 4x higher than coproporphyrin III. Analysis of urine porphyrin s show a normal level of coproporphyrin but the I isomer accounts for 80 of the total normally 25 . In post mortem autopsy , the liver will have a dark pink or black appearance due to pigment accumulation. There is plenty of canalicular multi drug resistant protein that causes bilirubin transfer to bile canaliculi . An isoform of this protein is localized to the apical hepatocyte membrane, allowing transport of glucuronide and glutathione conjugates back into the blood. The limitations of the assay depend upon the methodology of the test. HPLC High Performance Liquid Chromatography can detect certain genetic variants and potential liver diseases affecting CDT. Transferrin is a serum protein that carries iron through the bloodstream to the bone marrow, where red blood cells are manufactured, as well as to the liver and spleen. Structurally, transferrin is a polypeptide with two N linked polysaccharide chains. These polysaccharide chains are branched with sialic acid residues. Sialic acid is a monosaccharide carbohydrate . Various forms of transferrin exist, with differing levels of sialylation. The most common form is tetrasialotransferrin, with four sialic acid chains. In persons who consume significant quantities of alcohol usually more than 4 or 5 alcoholic beverages a day for two weeks or more , the proportion of transferrin with zero, one, or two sialic acid chains is increased. These are referred to as carbohydrate deficient transferrins . These carbohydrate deficient transferrins can be measured in the bloodstream, and are an important marker for alcohol abuse.
