Unreferenced date August 2010 Recurrent pyogenic cholangitis a.k.a. cholangiohepatitis is a condition that most commonly affects humans of East Asian origins. It is characterized by recurrent bouts of cholangitis in association with liver intrahepatic gallstone pigment stone s and intrahepatic jaundice biliary obstruction . It is also a common liver disease in cats. Pathogenesis In RPC the gallstones found within the biliary system are calcium bilirubinate stones or pigmented calcium stones. Calcium bilirubinate stones are prevalent in Asia and very rare in Europe and the United States . In addition to the presence of these friable concretions of various shapes and sizes within the biliary tree, the bile is often muddy in consistency and contains numerous fine particles of calcium bilirubinate. This differs greatly from cholesterol stones, which are common in Europe and the United States. Pure cholesterol stones contain 96 cholesterol whereas mixed cholesterol stones contain 71.3 cholesterol. The formation of calcium bilirubinate stones in RPC has been attributed to the high incidence of infection with Escherichia coli in the bile. In humans, the majority of bilirubin is excreted in the bile as bilirubin glucuronide. Calculus medicine Calculus bovis ref Ingredients, AN KUNG NIU HUANG WAN Bezoar Chest Functioning Pills , Peking Tung Jen Tang, Peking, China. 1980. ref , niu huang or ox bezoar s are dried gallstone s of cattle used in Chinese herbology , where they are claimed to remove toxins from the body. In Asian countries calculus bovis are harvested when cattle Bos taurus domesticus Gmelin are slaughtered. Their gall bladder s are taken out, the bile is filtered, and the stones are cleaned and dried. In western countries they are usually discarded. Calculus bovis have a color varying from golden yellow to brownish yellow. The shape of a stone is variable and depends on how it was formed, becoming spherical, oval, triangular, tubular or irregular. Since natural calculus bovis are scarce they can be very expensive. There are artificial calculus bovis used as substitutes. Infobox disease Name Porcelain gallbladder Image Porcelain gallbladder.png Caption Porcelain gallbladder on X ray DiseasesDB ICD10 ICD9 ICD9 575.8 ICDO OMIM MedlinePlus eMedicineSubj radio eMedicineTopic 569 MeshID Porcelain gallbladder is a calcification of the gallbladder believed to be brought on by excessive gallstone s, although the exact etiology cause is not clear. As with gallstone disease in general, this condition occurs predominantly in overweight female patients of middle age. It is a morphological variant of chronic cholecystitis. Inflammatory scarring of the wall, combined with dystrophic calcification within the wall transforms the gallbladder into a porcelain like vessel. Association with cancer Porcelain gallbladder is associated with gallbladder cancer , but the precise nature of the association is uncertain. Two review articles examined the association between cancer of the gallbladder and calcified gallbladder. One study reported an incidence of gallbladder cancer of about 1 in patients with calcified gallbladder. A KUB is a plain frontal supine X ray radiograph of the abdomen. It is often supplemented by an upright PA view of the chest to rule out air under the diaphragm or thoracic etiologies presenting as abdominal complaints and a standing view of the abdomen to differentiate obstruction from ileus by examining gastrointestinal air water levels . Uses Despite its name, a KUB is not typically used to investigate pathology of the kidneys, ureters, or bladder, since these structures are difficult to assess for example, the kidneys may not be visible due to overlying bowel gas. In order to assess these structures radiographically, a technique called an intravenous pyelogram was historically utilized, and today at many institutions CT urography is the technique of choice. KUB is typically used to investigate gastrointestinal conditions such as a bowel obstruction and gallstone s, and can detect the presence of kidney stones . The KUB is often used to diagnose constipation as stool can be seen readily. The KUB is also used to assess positioning of indwelling devices such as ureteric stents and nasogastric tubes. KUB is also done as a scout film for other procedures such as barium enemas. Projection It should include on the upright projections both right and left visualizations of the diaphragm. In at least one projection, the pubic symphysis must be present as the lower end of the area of interest. If the patient is large, more t ... more details
A zymogen or proenzyme is an inactive enzyme Protein precursor precursor . A zymogen requires a biochemistry biochemical change such as a hydrolysis reaction revealing the active site, or changing the configuration to reveal the active site for it to become an active enzyme. The biochemical change usually occurs in a lysosome where a specific part of the precursor enzyme is cleaved in order to activate it. The inactivating piece which is cleaved off can be a peptide unit, or can be independently folding domains comprising more than 100 residues. Although they limit the enzymes ability, these n terminal extensions of the enzyme or a prosegment often aid in the stabilizing and folding of the enzyme they inhibit. The pancreas secretes zymogens partly to prevent the enzymes from digesting protein s in the Cell biology cells in which they are synthesised. Enzymes like pepsin are created in the form of pepsinogen , an inactive zymogen. Pepsinogen is activated when Chief cells release it into HCl which partially activates it. Another partially activated pepsinogen completes the activation by removing the peptide turning the pepsinogen into pepsin. Accidental activation of zymogens can happen when the secretion duct in the pancreas is blocked by a gallstone resulting in acute pancreatitis . Fungus Fungi also secrete digestive enzyme s into the environment as zymogens. The external environment has a different pH than inside the fungal cell and this changes the zymogen s structure into an active enzyme.
