The cup to disc ratio is a measurement used in ophthalmology and optometry to assess the progression of glaucoma . The optic disc is the anatomical location of the eye s blind spot , the area where the optic nerve and blood vessels enter the retina. The optic disc can be flat or it can have a certain amount of normal cupping. But glaucoma, which is due to an increase in intra ocular pressure, produces additional pathological cupping of the optic disc. The pink rim of disc contains nerve fibers. The white cup is a pit with no nerve fibers. As glaucoma advances, the cup enlarges until it occupies most of the disc area. ref http www.agingeye.net glaucoma glaucomainformation.php AgingEye Times ref The cup to disc ratio compares the diameter of the Optic cup ophthalmology cup portion of the optic disc with the total diameter of the optic disc. A good analogy to better understand the cup to disc ratio is the ratio of a donut hole to a donut. The hole represents the cup and the surrounding area the disc. If the cup fills 1 10 of the disc, the ratio will be 0.1. If it fills 7 10 of the disc, the ratio is 0.7. A large cup to disc ratio may imply glaucoma or other pathology. ref http www.agingeye.net glaucoma glaucomaeyeexam.php ref However, cupping by itself is not indicative of glaucoma. Rather, it is an increase in cupping as the patient ages that is an indicator for glaucoma. Deep but stable cupping can occur due to hereditary factors without glaucoma. Pigment dispersion syndrome PDS is an affliction of the human eye eye that can lead to a form of glaucoma known as pigmentary glaucoma . It takes place when pigment cells slough off from the back of the Iris anatomy iris and float around in the aqueous humor . Over time, these pigment cells can accumulate in the anterior chamber in such a way that it can begin to clog the trabecular meshwork the major site of aqueous humour drainage , which can in turn prevent the aqueous humour from draining and therefore increases the pressure inside the eye. With PDS, the intra ocular pressure tends to spike at times and then can return to normal. Exercise has been shown to contribute to spikes in pressure as well. When the pressure is great enough to cause damage to the optic nerve , this is called pigmentary glaucoma. As with all types of glaucoma, when damage happens to the optic nerve fibers, the vision loss that occurs is irreversible and painless. This condition is rare, but occurs most often in Caucasians, particularly men, and the age of onset is relatively low mid 20s to 40s. For some reason, after 40 years of age, the syndrome lessens and stops. Most sufferers are nearsighted . There is no cure as of yet, but pigmentary glaucoma can be managed with eye drops or treated with simple surgeries. One of the surgeries is the YAG laser procedure in which a laser is used to break up the pigment clogs, and reduce pressure. Synthetic prostaglandin analogues are molecules which are manufactured to bind to a prostaglandin receptor . Wider use of prostaglandin analogues is limited by unwanted side effects and their abortion abortive potential. Uses Prostaglandin analogues such as misoprostol are used in treatment of duodenum duodenal and peptic ulcer gastric ulcers . They exert antisecretory and cytoprotective properties and are especially indicated for the elderly on continuous doses of NSAID s. Prostaglandin analogues can also be used in the management of open angle glaucoma . Ocular melanosis OM , also known as ocular melanocytosis or melanosis oculi , is a congenital disease of the human eye eye which affects about 1 in every 5000 people and is a risk factor for uveal melanoma . In dog s is found almost exclusively in the Cairn Terrier , where until recently it was known as pigmentary glaucoma see also canine glaucoma . The disease is caused by an increase of melanocyte s in the Iris anatomy iris , choroid , and surrounding structures. Overproduction of pigment by these cells can block the trabecular meshwork through which fluid drains from the eye. The increased fluid in the eye leads to increased pressure, which can lead to glaucoma . The nerve fibers forming the optic nerve exit the eye posteriorly through a hole in the sclera that is occupied by a mesh like structure called the lamina cribrosa . It is formed by a multilayered network of collagen fibers that insert into the scleral canal wall. The nerve fibers that comprise the optic nerve run through pores formed by these collagen beams. In humans, a central retinal artery is located slightly off center in nasal direction. The lamina cribrosa is thought to help maintain the pressure gradient between the inside of the eye and the surrounding tissue ref http www.springerlink.com index H28109374382X7T6.pdf Optic disc histomorphometry in normal eyes and eyes with secondary angle closure glaucoma by J.B. Jonas K.A. Koenigsreuther G.O.H Naumann in Graefe s Archive for Clinical and Experimental Ophthalmology 1992 230 134 139 ref . As a result of the Intraocular pressure it bulges slightly outwards. Being structurally weaker than the much thicker and denser sclera, the lamina cribrosa is more sensitive to changes in the intraocular pressure and tends to react to increased pressure through posterior displacement. This is thought to be one of the causes of nerve damage in glaucoma , as the displacement of the lamina cribrosa causes the pores to deform and pinch the traversing nerve fibers and blood vessels ref http www.pubmedcentral.nih.gov articlerender.fcgi?artid 1772339 Three dimensional analysis of the lamina cribrosa in glaucoma by J Morgan Davies N Taylor A R Hill P Aspinall C J O Brien A Azuara Blanco in Br J Ophthalmol. 2004 88 10 1299 1304 ref .
