    Pandy s test or Pandy s reaction is done on the Cerebrospinal fluid CSF cerebrospinal fluid to detect the elevated levels of protein s mainly globulins . This test is named after the Hungarian neurologist , P ndy K lm n 1868 1945 who developed this test in the year 1910. Principle Proteins globulin and albumin are precipitated by a saturated solution of phenol in water. The reagent used is phenol carbolic acid crystals dissolved in water or, pyrogallic acid or, cresol , usually termed as Pandy s reagent or Pandy s solution. Procedure One drop of CSF sample collected from the patient by lumbar puncture technique , is added to about 1ml of Pandy s solution. The turbid appearance signifies the presence of elevated levels of globulin protein in the CSF and is regarded as positive Pandy s reaction. The CSF from a normal adult shows no turbid precipitate indicates negative Pandy s reaction. Reactions and Results Proteins in the cerebrospinal fluid, normally albumin and globulin are present in the ratio of 8 to 1. Increases in protein levels are of diagnostic value in neurological diseases. The normal CSF is clear and transparent fluid. The Pandy s reaction makes it translucent or opaque. Positive test A bluish white streak of precipitated proteins is seen. The degree of turbidity depends on the amount of protein in the CSF. It can vary from faint turbidity mild to moderate elevation in CSF proteins to dense milky precipitate high protein content in CSF . Severe cases are known as cytokine storm s. The pathogenesis is that the antibody antibodies bind to the T cell receptor, activating the T cells before they are destroyed. The cytokine s released by the activated T cells produce a type of systemic inflammatory response syndrome systemic inflammatory response similar to that found in severe infection characterised by hypotension , pyrexia and rigor medicine rigors . The patient feels very unwell, as if in a high fever &ndash indeed, the cytokine release syndrome is effectively a type of non infective fever. Deaths due to cytokine release syndrome with OKT3 muromonab CD3 have been reported, and it can cause life threatening pulmonary edema if the patient is fluid overloaded. However, if treated appropriately it is usually not dangerous, just extremely unpleasant for the patient. The effect is greatly reduced by using low dose OKT3 e.g. 2  mg instead of 5  mg, slow infusion instead of rapid injection, intravenous administration of an anti histamine such as chlorphenamine and a corticosteroid such as hydrocortisone prior to starting the ATG OKT3 infusion, with further doses of anti histamine and steroid given during the infusion if necessary, Acetaminophine 500  mg by mouth 1 hour before infusion to prevent fever. Moreover, it is recommended that any overhydration to be corrected before the administration the 1st dose either by dialysis or with intravenous furosemide, to prevent the possible development of pulmonary oedema. Those with myeloma will typically display a rouleaux formation on a peripheral smear and a large globulin gap, indicative of a significant paraprotein load. While viscosity can be directly measured, results can take a few days to return and thus a high index of suspicion is required to make the diagnosis in a timely manner. If hyperviscosity is suspected, treatment may need to be started prior to obtaining the official viscosity level. Treatment Plasmapheresis may be used to decrease viscosity in the case of myeloma, whereas leukapheresis or phlebotomy may be employed in a leukemic or polycythemic crisis, respectively.
