orphan date October 2009 Visceral gout is a disease of birds in which kidney failure causes a build up or urate s in the internal organs, leaving a chalky white coating on them. Symptoms include anorexia symptom anorexia and emaciation. It is a problem common to caged birds. Vultures are particularly sensitive to poisoning by diclofenac, which leads to renal failure, visceral gout and death to them. Threat to vultures The collapse of the vulture population in India and Pakistan , to about a twentieth of their original numbers, has been caused by diclofenac , which is a non steroidal anti inflammatory drug NSAID . Diclofenac is given to working and ill farm cattle to reduce pain so that they can work on the land for longer. Farmers leave dead farm animals out in the open for the vultures to tidy up, and the vultures then feed from the carcasses of the cattle that die, and are poisoned by the diclofenac contained in the flesh they eat. Vultures are now not present in large enough numbers to tidy up all the dead animals. Instead, dead animals are now likely to rot in the open and be partly eaten by rats or wild dogs or other pests, causing major concern and human health hazards including risks of rabies . Meloxicam another NSAID has been found to be harmless to vultures and should prove an acceptable alternative to diclofenac. The Government of India has banned diclofenac, but it continues to be sold over a year later and is still a problem in other parts of the World. Notable direct causes of hyperuricosuria are dissolution of uric acid crystals in the kidney s or urinary bladder , and hyperuricemia . Notable indirect causes include uricosuric drugs, rapid breakdown of bodily tissues containing large quantities of DNA and RNA , and a diet high in purine . Drugs that may contribute to hyperuricosuria include allopurinol and uricase . Hyperuricosuria may be a medical sign of most likely first Gout very common Kidney stones of uric acid uric acid nephrolithiasis Acute uric acid nephropathy Acute renal failure Tumor lysis syndrome Fanconi syndrome Dent s disease very rare Acute vs chronic Acute medicine Acute hyperuricosuria is a common complication of tumor lysis syndrome. This syndrome appears not to contribute to gout and to uric acid nephrolithiasis, which is evidence that these two conditions have chronic, not acute, causes. Chronic medicine Chronic hyperuricosuria is associated with gout and uric acid nephrolithiasis. However, both conditions can occur in the absence of hyperuricosuria. Treatment of gout with uricosuric drugs can lead to uric acid nephrolithiasis. Treatment If neither hyperuricemia nor gout is present, then the risk of uric acid nephrolithiasis can be reduced by the use of antiuricosuric drugs. One should also consider eating a low purine diet. Additionally, making the urine pH more alkaline is protective. ref name pmid15875173 cite journal author Mattle D, Hess B title Preventive treatment of nephrolithiasis with alkali citrate a critical review journal Urol. Tophi are pathognomonic for the disease gout . Most people with tophi have had previous attacks of acute arthritis, eventually leading to the formation of tophi. Tophi form in the joints , cartilage , bone s, and other places throughout the body. Sometimes, tophi break through the skin and appear as white or yellowish white, chalky Nodule dermatology nodules . Without treatment, tophi may develop on average about ten years after the onset of gout , although their first appearance can range from three to forty two years. They are more apt to appear early in the course of the disease in people who are older in age. In the elderly population, women appear to be at higher risk for tophi than men. His first claim to distinction was his introduction of stoves made on a new plan, and intended for the heating of large public buildings. He afterward practised medicine and professed to be able to cure the gout without drugs, by muscular exercise alone. Whatever may have been the real efficacy of his method which seems analogous to the modern massage he was generally regarded as a quack because of the nature of his advertisement, abounding, as it did, in praise for himself.
