The term naturalhistory group refers to subjects in a drug trial that receive no treatment of any kind and whose illness is, as a consequence, left to run its natural course. The term stems from the Naturalhistory of disease naturalhistory of an illness , which is the course and outcome of that illness in the absence of treatment. First usage In 1863, Austin Flint 1812 1886 in his report of the first ever trial that directly compared the efficacy of a placebo treatment with that of an active treatment, spoke of the naturalhistory of an untreated disease . ref Flint 1863 , p.18. ref Third arm The naturalhistory group is often referred to as the third arm of a controlled drug trial, from the simple notion that a trial has three arms. The observed outcomes within this group are then compared with the outcomes manifested by a group that has been given the active drug, and with that manifested by a second group who have been given a dummy, placebo drug thus, the naturalhistory group is the trial s third arm . The naturalhistory of disease refers to a description of the uninterrupted progression of a disease in an individual from the moment of exposure to causal agents until recovery or death. Knowledge of the naturalhistory of disease ranks alongside causal understanding in importance for disease prevention and control. Naturalhistory of disease is one of the major elements of descriptive epidemiology . ref Bhopal, R. S. 2002 . Concepts of Epidemiology. Oxford Oxford University Press. http www.oup.co.uk isbn 0 19 263155 1 ref The iceberg phenomenon is a metaphor emphasizing that for virtually every health problem the number of known cases of disease is outweighed by those that remain undiscovered, much as the unseen part of an iceberg is much larger than the part that is visible above the water. The iceberg phenomenon thwarts attempts to assess the burden of disease and the need for services, as well as the selection of representative cases for study. This leads to what has been called the clinician s fallacy in which an inaccurate view of the nature and causes of a disease results from studying the minority of cases of the disease ref Morris, J.
