  1. Ureterosigmoidostomy

Unreferenced date December 2009 Interventions infobox Name Ureterosigmoidostomy Image Caption ICD10 ICD9 ICD9proc 56.71 MeshID OtherCodes A ureterosigmoidostomy is a surgery surgical procedure where the ureter s which carry urine from the kidney s, are diverted into the sigmoid colon . It is done as a treatment for bladder cancer , where the urinary bladder had to be removed. Rarely, the cancer presents in children between the ages of 2 & 10 yrs old as an aggressive rhabdomyosarcoma, although there are diagnoses of children as young as 3 months old. The procedure was also used several decades ago as a correctional procedure for patients born with bladder exstrophy . In the case of some bladder exstrophy patients, occasional bowel Fecal incontinence incontinence in this case, a mixture of urine and feces similar to diarrhea at night is one uncontrollable consequence. Another consequence of this procedure is an increased risk of kidney infections nephritis due to bacteria from faeces travelling back up the ureters reflux . Patients are commonly put on oral prophylactic antibiotics to combat infections in the uretery tract and the kidneys but this can lead to tolerance of the antibiotic, so over time the patient can build up tolerance to a large number of oral antibiotics, leading to a need for IV intravenous antibiotics administered while the patient is an inpatient. As well as this, the urine entering the colon can cause diarrhea and salt imbalance due to the sodium and chloride in the urine. Urea levels in the blood are higher due to urea crossing the colon wall. In the large intestine , sodium is swapped for potassium , and chloride for bicarbonate , this causes an acidosis and hypokalaemia . Many patients take sodium bicarbonate to combat this. Patients with ureterosigmoidostomy have an increased chance of developing colon cancer carcinoma of the colon after living with the modification for a number of years, on average 20 30 years after the operation, which can a ...   more details

The condition is often associated with hemolytic anemia , in which red blood cell s RBCs are destroyed, thereby increasing levels of free blood plasma plasma hemoglobin. The excess hemoglobin is filtered by the kidney s, which release it into the urine, giving urine a red color. Causes Acute glomerulonephritis Burn s Renal cancer Malaria Paroxysmal nocturnal hemoglobinuria Microangiopathies, e.g. Hemolytic uremic syndrome HUS , Thrombotic thrombocytopenic purpura TTP Transfusion reactions IgM autoimmune hemolytic anemia Pyelonephritis Sickle cell anemia Tuberculosis of the urinary tract March hemoglobinuria secondary to repetitive impacts on the body, usually the feet Athletic nephritis secondary to strenuous exercise Acute lead poisoning Complications Hemoglobinuria can lead to acute tubular necrosis which is a common cause of death of multi traumatic patients recovering in the Intensive care medicine ICU . Diagnosis The diagnosis is often made based on the medical history, blood samples, and a urine sample. 1. Symptomatic treatment

Unreferenced date December 2009 Symptomatic treatment is any medicine medical therapy of a disease that only affects its symptom s, not its cause, i.e., its etiology . It is usually aimed at reducing the signs and symptoms for the comfort and well being of the patient, but it also may be useful in reducing organic consequences and sequela e of these signs and symptoms of the disease. In many diseases, even in those whose etiologies are known e.g., most viral disease s, such as influenza , symptomatic treatment is the only one available so far. Examples of symptomatic treatments Analgesic s, for pain Anti inflammatory agents, for inflammation caused by arthritis Antitussive s, for cough Antihistaminic s, for allergy Cerebral shunt s, to alleviate hydrocephalus When the etiology for the disease is known, then specific treatment may be instituted, but it is generally associated to symptomatic treatment, as well. Symptomatic treatment is not always recommended, and in fact it may be outright dangerous, because it may mask the presence of an underlying etiology which will then be forgotten or treated with great delay. Examples Low grade fever for 15 days or more sometimes is the only symptom of bacteremia by staphylococcus bacteria. Suppressing it by symptomatic treatment will hide the disease from effective diagnosis and treatment with antibiotics . The consequence may be severe rheumatic fever , nephritis , endocarditis , etc. Chronic headache may be caused simply by a constitutional disposition or be the result of a brain tumor or a brain aneurysm . Finally, symptomatic treatment is not exempt of adverse effect medicine adverse effect s, and may be a cause of iatrogenic consequences i.e., ill effects caused by the treatment itself , such as allergic reaction s, stomach bleeding , central nervous system effects nausea , dizziness , etc. .
