Refimprove date June 2007 Image Acral gangrene due to plague.jpg thumb right 250 px A hand with acral gangrene due to bubonic plague plague . Acral necrosis is a symptom common in bubonic plague . The striking black discoloration of skin and tissue, primarily on the extremities acral , is commonly thought to have given rise to the name Black Death , associated both with the disease and the pandemic which occurred in the 14th century. The term in fact came from the figural sense of black , that is glum, lugubrious or dreadful. Clotting and bleeding beneath the skin causes an area of hemorrhage, the presence of red blood cells lying outside of capillaries, into the skin and subcutaneous tissue. In isolation, this is called an ecchymosis or bruise and may be the result of injury or illness. However, acral necrosis occurs when blood supply is disrupted for prolonged periods, blackening and damaging the affected area and surrounding tissue. With appropriate medical treatment, areas with Acral necrosis may be successfully restored to function and lead a normal life. Untreated cases can lead to death. Liquefactive necrosis or colliquative necrosis is a type of necrosis which results in a transformation of the tissue into a liquid viscous mass. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 15 ref Often it is associated with focal bacterial or fungal infections. In liquefactive necrosis, the affected cell is completely digested by Hydrolysis hydrolytic enzymes, resulting in a soft, circumscribed lesion consisting of pus and the fluid remains of necrotic tissue. Dead leukocytes will remain as a creamy yellow pus. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 15 ref After the removal of cell debris by white blood cells , a fluid filled space is left. It is generally associated with abscess formation and is commonly found in the central nervous system . In the brain For unclear reasons, Hypoxia medical hypoxic death of cells within the central nervous system also results in liquefactive necrosis. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 15 ref This is a process in which lysosome s turn tissues into soup as a result of lysosomal release of digestive enzymes. Loss of tissue architecture means that the tissue is essentially liquefied. This process is not associated with bacteria action or infection. The affected area is soft with liquefied centre containing necrotic debris. Later, a cyst wall is formed. Microscopically, the cystic space contains necrotic cell debris and macrophage s filled with phagocytosed material. The cyst wall is formed by proliferating capillaries , inflammatory cells, and gliosis proliferating glial cells in the case of brain and proliferating fibroblast s in the case of abscess cavity. In the lung Liquefactive necrosis can also occur in the lung. ref name urlCell Injury cite web url http library.med.utah.edu WebPath CINJHTML CINJ020.html title Cell Injury format work accessdate 2009 05 15 ref ref name pmid19082781 cite journal author Quaia E, Baratella E, Pizzolato R, Bussani ... more details
