  1. Pittsburgh knee rules

The Pittsburgh knee rules are medical rules created to ascertain whether a knee injury requires the use of Radiology X ray to assess a fracture . Criteria Blunt trauma or a fall as mechanism of injury AND either of the following Age younger than 12 years or older than 50 years Inability to walk four weight bearing steps in the emergency department. If the patient satisfies the above criteria, they should receive an X ray to assess for a possible fracture. Accuracy The sensitivity of using the Pittsburgh Knee rules is 99 with a specificity of 60 . That means the use of the above rules has a false negative result of 1 and a false positive result of 40 . The false positive result is less important as if the patient is positive, they should receive an Xray to assess for a possible fracture which has a much higher specificity. The use of the Pittsburgh Knee rules reduces the use of knee radiographs by 52 . ref Tandeter HB, Shvartzman P. Acute knee injuries use of decision rules for selective radiograph ordering. Examples include surgical emergency emergency trauma surgery conducted outside the operating room such as on scene amputation surgical amputation , cricothyroidotomy , or thoracotomy , or administration of medication such as certain antibiotics and chemotherapy drugs at dosage levels high enough to potentially cause serious or fatal adverse effect side effects . ref Merriam Webster s Medical Dictionary , 3rd Edition, Merriam Webster, Inc. ref ref Heroic Measures , Dr. Charles Bardes M.D, AGNI Online Essays, http www.bu.edu agni essays online 2006 bardes heroicmeasures.html ref Cardiopulmonary resuscitation is a particularly well known heroic measure vigorous chest compressions often result in bone fracture fracturing one or more of the patient s ribs , but since the alternative is certain death , the technique is accepted as necessary. Patients with advanced AIDS and concomitant pneumocystis pneumonia PCP are in serious danger of acute respiratory distress syndrome ARDS . A heroic rescue involved using the chemotherapeutic drug trimetrexate but that destroys the marrow as well as the PCP. To overcome this barrier, leucovorin can protect the marrow and allow the trimetrexate to fight the PCP. Severe haemorrhage in trauma diminishes oxygen delivery, causing the patient s body temperature to drop hypothermia . This in turn can halt the Coagulation coagulation cascade , preventing blood from clotting coagulopathy . In the absence of blood bound oxygen and nutrients hypoperfusion , the body s cells burn glucose anaerobic respiration anaerobically for energy lactic acidosis , which in turn increases the blood s acidity metabolic acidosis . Such an increase in acidity can reduce the efficiency of the heart muscles myocardial myocardial performance , further reducing the oxygen delivery and hence triggering a deadly cycle.
